Care Coordinators

Our Practice-Based Care Coordinators Sarah, Hayley, Karen, Claire, Hayley, Sharon, Katie, are available to support patients with Cancer, Dementia/Frailty, Learning Disabilities, after being discharged from Hospital, and Safeguarding.

They are based between the eight GP surgeries within the Stockport East & South Primary Care Network (SES PCN).
