An alternative option available is to register for access to Patient Access instead, following the process below.
To be able to access Patient Access for the first time you will need to have an individual access and ID code. Please contact Reception, who will be able to issue you with this. All new patients registering with us will automatically receive a logon and password.
Complete our Online Services Registration Form and return to the Practice along with the correct forms of identification (see below).
Alternatively, you can complete the form electronically and return to the Practice by email, accompanied with a photograph of each form of identification (as specified below), along with a photograph of you holding the Photographic ID. Please email the ID along with the Registration form to [email protected]
