Viewing your records via NHS App
Patients can view their medical records in real-time using the NHS App. There is a separate request form to access NHS App (Online Services). Please click here for more information on how to access the NHS App.
Data Subject Access Request (DSAR)
In addition to NHS App, and in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018, patients may request to access / copies of their medical records by completing a Data Subject Access Request (DSAR).
We will require completion of DSAR Form (below) and will need verification of your identity before releasing any information. This will must be two forms of identification; one form of photographic identification and a second form as proof of address.
DSAR Process

- Return the completed form to the practice via our Online Triage System by clicking here or via Email to:
gmicb-sto.MarpleGuywoodSupport@nhs.net - You will need to attach a copy of your identification documents. (If you are emailing to the practice, please include a photo of you holding your Photographic Identification.)
DSAR requests received with the correct forms of identification will be reviewed and processed within one calendar month following receipt of the request, however if it is a complex request or there are a significant number of requests, the response can be extended by a further two months.
