If you require a sick note for a condition after you have self certified for the first 5 working days, you will need to see a GP at the practice.

If you have already had a sick note issued by a GP, you may be able to request another one without an appointment.

To do this follow the below:

1. The day your sick note runs out, contact the surgery (our phones are less busy after the morning surgeries or why not email Marplecottage.reception@gp-p88006 )
   
   
2. We will need the following information:
   
   Your full name and address
   The date you want your sick note to start from
   Your medical condition, which is on your current sick note
   The duration for which you need it eg: one week
   
   
3. If you need a signing off note, you can request this by ringing, emailing or calling in, giving your name and address and the date you wish to return to work.
   
   
4. Once your request has been agreed by a GP your sick note will be ready
   within 2 working days. If for whatever reason your request is declined,
   someone from the surgery will contact you to inform you of this and to book
   a further appointment.
