More than fifteen million people in this country report living with a long-term condition. Long-term conditions are conditions that cannot, at present, be cured, but can be controlled by medication and other therapies.
Many live with a condition that limits their ability to cope with day-to-day activities. For some people, especially older people and those who have more than one condition, discomfort and stress is an everyday reality.
The NHS Improvement Plan, published in June 2004, set out the Government’s priority to improve care for people with long term conditions by moving away from care by specialists in hospitals to expert care delivered close to home by doctors and nurses at your general practice.
Helping patients to self-manage is an
important part of this service too.
