Based upon the successful pilot, which ran in Stockport in 2004 and to meet the challenges set out in the NHS Strategy 2004 ‘Managing Long Term Conditions’, the Active Case Management Team (ACM) was established.
This service has just started for patients in Marple. The ACM team will be working with Marple Cottage Surgery clinical staff to identify patients who may benefit from a higher level of clinical and/or social attention to maintain there well-being and remain as independent as possible.
Aims and Objectives of the service
- Develop and implement a proactive approach to identifying clients
- Develop individualised care package following detailed assessment
- Provision of long-term ongoing support
- Reduce the number of inappropriate admissions to secondary care sectors
- Reduce the amount of inappropriate high use and dependency upon health & social care services
- Improve quality of life
- Develop independence and self-care and self-management of client group
